Tumescent means to become swollen. Tumescent liposuction involves the use of a diluted solution made up of epinephrine and lidocaine. This solution is injected into the fat underneath the skin, causing the area to swell and firm up. (*The surgeon then uses a small surgical instrument to suction away the excess fatty deposits).

History

The Tumescent Technique was developed in 1985 by Dr. Jeffrey A. Klien, M.D., a dermatologist from California. Prior to 1985, liposuction was quite a bit more risky, often causing so much blood loss that patients frequently required a blood transfusion. Vasoconstriction, resulting from the dilute epinephrine used in the Tumescent Technique, has virtually eliminated blood loss during the liposuction procedure. This method is now considered the safest, most effective form of liposuction.

Benefits

Tumescent liposuction can significantly reduce some of the common side effects of older procedures. The epinephrine/lidocaine solution temporarily reduces the size of the capillaries, which results in significantly less blood loss. The majority of patients only lose about 15-30 ml of blood. It also helps reduce pain, bruising, and swelling post-surgery.

Risks

As with any surgery, there are risks associated with the tumescent technique; although the risks are minimal. If the amount of lidocaine in the solution is too high, a patient could get lidocaine toxicity. There is also a risk of fluid accumulation if too much solution is injected. These are risks a prospective candidate should discuss with his/her doctor prior to surgery.

Ideal Candidate

This surgery is not meant as a substitute for weight loss. The ideal candidate should have already achieved an ideal body weight and have elastic and firm skin. The procedure is most effective on a few problem areas which are difficult to eliminate with diet and exercise alone.

Dr. Klien’s discovery has significantly reduced the risks associated with liposuction. The Tumescent Technique has eliminated the need for blood transfusions common in older forms of liposuction procedures. In addition, these surgeries do not require hospitalization and general anesthesia is not used. Patients now recover faster, experience less pain, and receive optimal cosmetic results. VASER® Lipo System

By combining the Tumescent Technique with the VASER® Lipo System, patients can anticipate fast, consistent results and a quick recovery. This system is the only one available with the ability to differentiate between the targeted fat and the surrounding tissue, which means the surgeon can safely break up the unwanted fat while preserving the connective tissue, nerves, and blood vessels.

VASER® stands for Vibration Amplification of Sound Energy at Resonance. It was designed by Sound Surgical Technologies, LLC and received FDA clearance for body contouring in 2002. Although fairly new, this procedure is comparatively safe with thousands performed worldwide without serious complications.

How It Works

First, the tumescent fluid is injected into the fatty tissue around the area to be suctioned. The VASER® system then uses ultrasonic waves to cause the fat cells to break apart and emulsify. This allows for easy removal of the fat using either a cannula or a massage.

Areas That Can Be Treated

This technique can be used anywhere where there are small pockets of extra fat. The most common areas include: abdominals, thighs, knees, buttocks, arms, love handles, breasts, hips, back, neck and chin.

Amount of Fat That Can Be Removed

It is important to remember that liposuction is not a substitute for weight loss. The amount of fat that can typically be removed is equivalent to the amount of loose hanging fat you could grab on your belly if you stood up and bent forward. Some fat in the area is left untreated, so with significant weight gain this remaining fat will expand. It is important to maintain healthy eating and exercise habits in order to retain optimal results.

Results

As with all cosmetic surgeries, results will vary per patient. Most patients start to see results immediately; however, final results may not be seen for up to 6 months. Results vary depending on the amount of fat removed and the number of sites treated.

Again, this is not a substitute for weight loss. The ideal candidate is at a healthy weight seeking to treat stubborn areas difficult to lose with diet and exercise alone.

What bothers me as a practicing plastic surgeon, is that I see cases where liposuction is NOT done correctly and the results can be terribly traumatic for the patient. The purpose of this article is to help you avoid the issues that commonly bring about poor results.

Part of the problem is that too many unqualified people are practicing liposuction while making unrealistic promises. With the huge public demand, many of these pop-up Lipo Centers do a thriving business. Establishing such a business has become easier with advanced tools and technology.

With the advent of new technology, liposuction has expanded beyond the realm of plastic surgery into other medical specialties. Nowadays, a number of general practitioners with no surgical training are performing liposuction in their offices under local anesthesia, loudly promoting it as “safe” liposuction, “non invasive” liposuction or even “non surgical” liposuction. They often deceivingly understate the recovery time by giving it such slogans as Lunch-time Liposuction.

Simple truth is, liposuction is surgery and it is invasive, particularly when some form of additional energy is utilized such as the case with Laser Liposuction. (Sometimes referred to as Smart lipo or Slim lipo, Ultrasonic liposuction by Vaser or Lysonics).

On a technical level, the laser energy disrupts the fatty tissue more aggressively than non-laser lipo and theoretically, it can be extracted more efficiently. The unspoken caveat is that laser lipo can also cause too much tissue disruption, which leads to deformity, particularly in the hands of untrained or unqualified practitioners.

The popularity of Smart Lipo, and the great marketing campaigns promoting it, has led to a wave of unfavorable results that are very hard, if not impossible, to correct. The irony is that the unqualified practitioners who produce these deformities are totally incapable of correcting their shoddy work.

To compound the problem, many patients with the unfavorable results are ashamed to come forward. I truly wish more of them would speak up so it could help future patients be aware and prevent such disasters. I see it in my office though…way too often.

Sucking a little bit of fat might seem like a no-brainer, anybody can do it! The truth is, it takes meticulous study, training and experience to do it well. It takes knowledge of what constitutes an attractive human contour, how to decide when a patient is a good candidate or not, and whether the patient’s skin will allow for a good outcome.

With laser-lipo in particular, it takes a sense of what is too much energy, too much suction and too much compression during recovery to prevent permanent damage.

The best results are always obtained during a first surgery done properly, not when trying to correct damage during a second surgery.

At the 2009 ASAPS (American Society for Aesthetic Plastic Surgery) meeting in Las Vegas, emphasis was given to correction of deformities after liposuction. The problem is definitely big enough to warrant such attention.

Talented experts presented difficult cases that were corrected with the use of fat grafting. This type of correction becomes even more challenging when the patient had all the fat already removed in the original defective procedure.

My recommendation is to research thoroughly the competency of the practitioner doing your liposuction. Visit more than one expert, get references from friends and other people that have used them and are satisfied with the results. Ask your doctor if he/she knows how to correct bad results. Don’t forget to ask for before and after photos that clearly show improvement.

If you have been the victim of poorly done liposuction, my heart goes out to you. Please know that your situation can be made better if you obtain a high quality doctor.

If you are contemplating liposuction, I warn you to not be too enchanted with the extreme promises made in some advertisements. In the hands of a qualified doctor, liposuction is an exciting, beneficial procedure. Just make sure your doctor is up to the job.

What is it?

Stem cell breast augmentation is a procedure based on regenerative medicine and involves the collection of stem cells from your own adipose (fat) tissue and the re-injection of those cells, together with purified adipose tissue into the breast. The stem cells maintain the viability of the fat that has been injected and create an infrastructure for it to survive. The fat tissue then integrates naturally with the surrounding breast tissue, resulting in permanently bigger, firmer breasts. It is possible to gain 1-2 cup sizes per treatment depending on how much fat is harvested. Using the patients own cells represents a highly safe treatment with no concerns of foreign body reaction, such as encapsulation, allergies or tissue rejection.

Stem cells are a particular type of cell that can differentiate into various other cells when placed in the correct environment. They also release growth factors which stimulate blood vessel production which maintains the viability of the surrounding tissue.

Stem cells have of recent attracted significant interest in several different medical specialities including heart attacks, strokes, heart failure, rheumatoid arthritis, degenerative bone disease, diabetes, Parkinson’s, MS, liver cirrhosis, skin grafts and scar revision.

Where is the fat taken from?

Stem cells are harvested from the patient’s own fat during our VASER Liposelection procedure. Stem cell breast augmentation therefore is to be used in combination with ultrasound assisted liposulpture. A modified liposelection procedure is performed under local anaesthetic and the tissue is then processed in a machine which concentrates the stem cells. Once this procedure is complete the concentrate is then combined with more fat tissue and re-injected into the breasts through tiny little puncture holes, thus leaving no scars. The most common areas where fat is harvested from are abdomen, outer thighs and flanks.

How are the cells administered into the breasts.

Equal amounts of cell concentrate and fat tissue are injected into both breasts to give them a fuller and firmer shape. The procedure is performed under local anaesthetic and administration is via a small cannula which leaves no scar. The fat tissue gives the breast a natural look and feel.

What are the risk factors?

As with any other minimally invasive procedure there is a small risk of infection, bleeding, bruising. As the fat is your own tissue the chances of allergic reaction, rejection and capsule formation are almost negligible compared to traditional breast augmentation techniques.
