4 Facts About Liposuction Before You Talk to Any Cosmetic Surgeon

Liposuction is the technique used to suction the fatty deposits from certain body parts that refuse to go away in spite of dieting and exercising. It is now one of the most sought after methods of removing excess body fat. Although a relatively safe procedure, liposuction methods are constantly being fine tuned to improve the results. The most common areas of fat removal are the thighs, the stomach and the buttocks.

History

Several people claim to have invented the liposuction procedure, which makes the history a little difficult to decipher. There is some sketchy information available stating that liposuction was first attempted in the early 1920′s with less than satisfactory results. Supposedly, Charles Dujarier, a surgeon in France, performed the procedure on a model from Paris who later died as a result of gangrene complications. This ended the method of removing fat by suction for many years.

Liposuction again came to the forefront in the year 1974. This time it was reinvented by Dr. Georgio Fischer and his son, Dr. Georgio Fischer, Jr., both Italian gynecologists. Using an electric rotating scalpel they were able to cut fat tissue out of the body through small incisions that were made. The tool used was basically an oscillating blade inside a cannula, or a thin tube made from metal. The fat cells were essentially destroyed and along with bits of tissue and blood, were removed from the body by suction. Not as evolved as it is today, the method nonetheless paved the way for modern liposuction methods.

Side Effects

Unfortunately, many early attempts at liposuction resulted in patients experiencing side effects such as profuse bleeding, numbness that became permanent and less than perfect results in the area where the fat had been removed. Different surgeons began to develop their own special methods of liposuction techniques. During the 70′s and 80′s liposuction was constantly being revised and fine tuned. Different equipment was used including various oscillating blades inside metal cannulas, and even bladeless cannulas were tried. The downside to these attempts was that the patient was the one who became a guinea pig for these newer methods, and suffered the consequences if the procedure didn’t work as expected.

Advancement

In the year 1977, the blunt tipped cannula was introduced by Dr. Yves-Gerard of France. Less bleeding occurred, and nerves and blood vessels weren’t damaged in the way of the older methods. It made it much easier for the fatty tissue to be manipulated. The recovery time after surgery was much shorter, and post-operative discomfort was reduced. With much less loss of blood there were far fewer complications to be concerned about. The liposuction procedures were far from perfect as of yet, but the results were better than they had been in earlier years. The surgery continued to be performed in this manner for several years.

In 1985, liposuction underwent a new change with the invention of the Tumescent method developed by Dr. Jeffrey Kline from California. Dr. Kline, a dermatologist, figured out how to enable patients to have liposuction with only local anesthesia. There was much less bleeding, and the skin didn’t appear to be as dimpled as it once would have been. The cannula was much smaller and more slender than the one used prior to this time. The surgeon was better able to ‘sculpt’ the area. Microcannulas used in Tumescent liposuction remove less fat per minute that a larger cannula, but the fat removal is much smoother and the volume greater.

In 1987, UAL liposuction was developed by Dr. Michelle Zocchi. UAL means ultrasonic assisted liposuction, which offered a much more refined method of fatty tissue removal. The cannula was further reduced in size by 1998, being about the diameter of a small straw. This allowed for the procedure to be done much less invasively and resulted in a more satisfying appearance when the surgery was finished. The bleeding was minimal and recovery time was reduced to a matter of a few weeks.

In the year 21st century, liposuction is a relatively routine procedure performed on a daily basis for both women and men alike, worldwide. The procedure is performed in a hospital or a clinic by a qualified surgeon. Either general or local anesthesia is used for sedation or numbing.

Liposuction and Body Contouring

Liposuction continues to the most common plastic surgery procedure all over the world, and also in India. It is basically a permanent removal of fat cells by the use of a suctioning device. It is a safe procedure, with minimum down time, and fast recovery. It has been shrouded in small controversies, and bad results, but this has been due to the procedure being done by untrained personnel, and other medicos who are not Cosmetic Surgeons. So it is important that a potential client look through the profile of their treating physician and asses really whether that person is actually capable of delivering all that he promises. Do not be led away by claims that they can remove 10 kgs or 12 kgs of fat and make you super thin. Please remember that the safe limits of liposuction are between 3-5% of the total body weight.

Procedure

The procedure involves infiltration of fluid into the areas to be liposuctioned. This has multiple benefits. It helps to loosen the fat, reduces the bleeding dramatically, and allows pain control after surgery as well. Usually I would infiltrate fluid in a ratio of 1:1 with the amount of fat that I plan to aspirate, ie, if I feel that I will aspirate about 1000 cc of fat from an area, I would like to infiltrate less than 10000 cc of the fluid in that area. I actually infiltrate much less than that, only about 0.5 :1 ratio. This has the benefit that I never face the risk of fluid overload ( potentially dangerous condition, where there is too much water in the body, and the lungs get filled with water). The other benefit is that, what is aspirated is pure fat, and not water mixed with fat. This gives an exact estimate of the amount of fat taken out. We surgeons have a bad habit of including the amount of fluid aspirated into the total volume taken out also, so that we can boast of having done large liposuctions. What is important is not the amount taken out, but how much of actual fat there is.

The procedure is done with small incisions over selected areas of the body, after first discussing with the patient, and planning the site of the incision. These incisions generally are not visible and heal very well. They are only about ½ cm in length. There may not be a need to admit the patient in the hospital, so that the patient can go home the same day.

Precautions

After a liposuction, there is a need to wear specially designed pressure garment, over the areas liposuctioned to allow better skin redraping. This may take up to 6 weeks. Any anticoagulant use such as aspirin should be stopped 1 week prior to surgery; smoking should also be stopped 2 weeks before to 2 weeks after surgery for best results.

Newer technology

Power liposuction, vibro-liposuction, VASER, and laser liposuction are some of the newer innovations in this field.
In Power and Vibro Liposuction the mechanical effort to carry out liposuction is reduced by the use of special machines, which produce a to and fro rapid motion, thus reducing mechanical fatigue and making the procedure faster. However these are significantly more expensive than conventional liposuction.

VASER is the use of internal ultrasound device to melt fat. The advantage is that this is significantly faster, and allows the surgeon to work closer to the skin, and thus better skin draping is achieved, and thus better figure control is achieved. However this is a very expensive device, and expects the surgical cost to double if you ask for a VASER.
Laser lipo allows very good redraping action on the skin; however its role in actual fat reduction is a bit controversial. Thus this has not found use in general practice.

What is Liposuction?

Although there were European surgeons practicing some form of surgical fat removal as far back as the 1920′s, modern liposuction has been performed for over 35 years. It was first invented in Italy in 1974 by Dr. Giorgio Fischer, an American working in Rome. During liposuction (also called lipoplasty) surgery, excess fat deposits are removed from beneath the skin using a surgical instrument called a cannula, which essentially is a hollow tube. The movement of the cannula, guided by the surgeon, breaks up fat deposits which are then vacuumed out of the body through the cannula opening.

When the term “traditional liposuction” is used, it refers to the technique used to break up the fat deposits – namely, the movement of the cannula and the removal by suction using a vacuum device called an aspirator. Traditional liposuction is also referred to as SAL, or Suction-assisted liposuction.

In more recent years the technology has evolved and more options are now available to liposuction surgeons. One of these options, developed in the 1980′s, makes use of sound waves to help break up the fat deposits. Referred to as “ultrasonic liposuction”,or UAL, this technique still utilizes a cannula and aspirator; however the cannula is a specialized version which produces ultrasound vibrations within the body. Patents for ultrasonic liposuction devices were first issued in 1987. The fat is liquefied by the sound waves rather than broken up simply by the movement of the cannula. Liposuction Methods

The amount of solution injected is the difference between “wet” and “tumescent” techniques; with the “tumescent” technique up to 5 times the amount of fluid is injected. This makes the target area firm and inflated. Using the tumescent technique aids the surgeon to extricate the fat in a smoother, more controlled manner. In addition, there is less chance of damage to surrounding tissues, and patients typically recover in a shorter period of time

The tumescent technique was invented by Dr. Jeffrey Klein, a California cosmetic surgeon, in 1986. Dr. Klein pioneered the use of tumescent solution in liposuction surgery. His mixture includes:

  • Adrenaline, to reduce bleeding by temporarily closing capillaries
  • Lidocaine for local anesthetic value
  • Bicarbonate to reduce acidity, thereby reducing the discomfort of the patient

These ingredients are combined with a saline solution and injected into the target area.
